REDUCE RE-HOSPITALIZATION We are dedicated to working with our local healthcare community to decrease hospital readmission and reduce medical costs. We see patients and their families daily for a minimum of three days following a hospital discharge, and our transition support team will “tuck in” patients every night with additional phone calls to ensure compliance and prevent re-hospitalization. Progressive’s Cardiac Care Program is designed to empower cardiac patients to effectively manage their disease, reduce hospitalizations and health care costs, as well as educate the community about heart disease. Our infusion nurses are experts in the administration of medication through a needle or catheter for severe conditions that cannot be treated effectively by oral medications. We can assist with antibiotic therapy, hydration, nutritional support, pain management, or anything else ordered by the physician.
