INDICATORS FOR NEEDING HOME HEALTH CARE You may need skilled nursing care if you notice a decline in your abilities or changes in your activities of daily living (ADLs). Other indicators for skilled home health care include:

REDUCE RE-HOSPITALIZATION We are dedicated to working with our local healthcare community to decrease hospital readmission and reduce medical costs. We see patients and their families daily for a minimum of three days following a hospital discharge, and our transition support team will “tuck in” patients every night with additional phone calls to ensure compliance and prevent re-hospitalization. Progressive’s Cardiac Care Program is designed to empower cardiac patients to effectively manage their disease, reduce hospitalizations and health care costs, as well as educate the community about heart disease. Our infusion nurses are experts in the administration of medication through a needle or catheter for severe conditions that cannot be treated effectively by oral medications. We can assist with antibiotic therapy, hydration, nutritional support, pain management, or anything else ordered by the physician. Progressive Home Health has an experienced wound care team consisting of all registered nurses (RN) that are KCI Tier 2 Wound Vac Certified in advanced wound care techniques. CARDIAC CARE PROGRAM
