Progressive’s Home Health Care is delivered by professional nurses (RN/LPN) under a physician’s order. Our care plans target the needs of each client with a strong emphasis on positive patient outcomes and goals. Our home health services can be needed following a hospitalization or when dealing with an acute or chronic illness that requires ongoing monitoring by a professional.

We are dedicated to working with our local healthcare community to decrease hospital readmission and reduce medical costs. We see patients and their families daily for a minimum of three days following a hospital discharge, and our transition support team will “tuck-in” patients every night with additional phone calls to ensure compliance and prevent re-hospitalization.

Our infusion nurses are experts in the administration of medication through a needle or catheter for severe conditions that cannot be treated effectively by oral medications. We can assist with antibiotic therapy, hydration, nutritional support, pain management or anything else ordered by the physician.
Progressive Home Health has an experienced wound care team consisting of all registered nurses (RN) that are KCI Tier 2 Wound Vac Certified in advanced wound care techniques.
