How does it work?
When a referral is made we schedule a home visit where we install the equipments and teach the client or family how to use it.
Every day the client take own measurement per Tele- monitor and automatically sent the information via phone line to Progressive Home Health& Hospice.
Our Tele-monitoring RN will review this information and initiate the clinical interventions. The RN will also report the information to the physician.
The wireless system transmits real time vital data into our centralized communication station including patients: Weight, Glucose levels, Oxygen saturation, Blood pressure, vital signs, peak flow meter, PT/INR meter, Enhanced SPO2 algorithm.
