Progressive’s Home Health Care is delivered by professional nurses (RN/LPN) under a physician’s order. Our care plans target the needs of each client with a strong emphasis on positive patient outcomes and goals. Our home health services can be needed following a hospitalization or when dealing with an acute or chronic illness that requires ongoing monitoring by a professional.

Transition Care

Progressive now offers a Transition Care Coordinator RN to ensure the continuity of care for patients transitioning from a facility to home care or hospice. This RN meets patients in the facility and ensures supplies and equipment needed for home care are delivered in a timely manner.

Indicators for In-home Care

You may need in-home care if you notice a decline in your abilities or changes in your activities of daily living (ADLs). Other indicators for in-home care include:

  • Medical Treatment Regiment Change
  • New Diagnosis
  • Frequent Hospitalizations
  • Disequilibrium / Fall Risk
  • Infection / IV Antibiotics
  • Intractable Pain

Reduce Re-hospitalization

We are dedicated to working with our local healthcare community to decrease hospital readmission and reduce medical costs. We see patients and their families daily for a minimum of three days following a hospital discharge, and our transition support team will “tuck-in” patients every night with additional phone calls to ensure compliance and prevent re-hospitalization.

Cardiac Care Program

Progressive’s Cardiac Care Program is designed to empower cardiac patients to effectively manage their disease, reduce hospitalizations and health care costs as well as educate the community about heart disease. We provide the following cardiac care services:

  • Medications Education & Monitoring
  • Home Exercise & Diet Education
  • Post Cardiac Artery Bypass Graft Care
  • Congestive Heart Failure Management
  • Home Intravenous Therapy
  • Care for End Stage Cardiac Disease

Wound Care Specialists

Progressive Home Health has an experienced wound care team consisting of all registered nurses (RN) that are KCI Tier 2 Wound Vac Certified in advanced wound care techniques.
