Hospice Program
Program Goals:
Provide an individualized program of physical, emotional, spiritual, and
practical care for patients in the last phases of a life-limiting
illness.
Emphasize control of pain and other symptoms.
Be reflective of a spirit and idea of caring emphasizing dignity and
comfort for the dying.
Make it possible for the patient to remain independent for as long as
possible in familiar surroundings.
Services Provided Include:
Skilled nursing 2-3 times per week
Skilled nurse continuous care when applicable
Palliative care
Bereavement services up to 12 months
Chaplain visits
Registered dietician
Medical social worker
Home health aide for bathing and personal care
No medication co-pays
Medication related to terminal diagnosis are covered
Therapy services (OT, ST, PT)
Durable medical equipment and supplies are covered
Hospice services are provided any place a patient chooses to call home
including assisted living and nursing homes.

Volunteer staff services
Staff Qualification:
Our Hospice Program utilizes an inter-disciplinary group approach of
physicians, nurses, social workers, aides, homemakers, volunteers,
spiritual counselors, bereavement counselors, etc. All staff members
must have extensive end-of-life experience and be orientated to our
Hospice philosophy.
